IMHO, most controls utilised are from the bottom two levels of the Hierarchy of controls usually because of ease, simplicity and cost. How many come from the top three levels? — KeithH

Why does a replacement need to be proposed to stop using something when it has a detrimental affect - in other words (in some / a lot of cases) doing nothing is better than using a risk matrix.I haven't come across anything else that would work any better.
