  • Drug testing: time to abandon it being limited to safety-sensitive areas only?
If we include alcohol in harmful substance using behaviours then over 75% of the population could be idetified as deviance prone.
  • Drug testing: time to abandon it being limited to safety-sensitive areas only?
    An interesting perspective from europe

  • Drug testing: time to abandon it being limited to safety-sensitive areas only?
Is there any evidence that drug testing has reduced workplace deaths and injuries? A look at ACC stats would indicate no! Over 10 years of drug testing safety sensitive workers and people are still dying and being injured at alarming rates.
  • Near miss reporting
Reporting near miss is a demand from management. We use unsafe acts and unsafe conditions along with near miss. using this approach empowers workers to report 'that was close' I nearly got hurt because of an unsafe condition. Managers then respoond by improving conditions not investigating workers. Demands from management will never work. Empower workers to get results.
  • What do you measure and how do you measure it
Many Management positions include a vehicle. Show true leadership and commitment on carbon reduction and drive an EV instead of a Ford Ranger.
  • ICAM Investigation Course
Proper task preparation not done because of a low perception of the risks by managers and supervisors preparing the task activity.
  • ICAM Investigation Course
No experience with ICAM but we use six sigma tools like 5 why's and fish bone analysis. all investigations using this approach get beyond operator error and dig deep to the route cause. This usually leads to 'proper task preperation was not done'.
  • Risk Assessment Matrix
We use scoring and traffic light colours.
Green < 20 Very low Limited risk: acceptable
Yellow 20 - 70 Possible Monitoring: pay attention
    Orange 70 - 200 Significant Measures required
    Brown 200 - 400 High Immediate improvement
Red > 400 Very high Stop Activity

Assess risks without controls - reassess with controls, score must be less than 70
A good source is ISO 12100-2010 Safety of machinery - General principles for design - Risk assessment and risk reduction.
  • Fatigue Managment
Yes, Standard site contracts. believe me that project managers choose to ignore these rules when it suits.
  • Fatigue Managment
Here's one from a typical green field factory contruction site:

    Work Hours
In accordance with the Fatigue management policy the hours of work should follow:
    • 12 hour day’s
    • No more than 70 hours per week
    • No more than 17 days continuous in any 21 days
    • [Provide safe transport to and from accommodation]

    Extended work hours management:
    • No more than 15 hours in any 24 hours
    • Only 1 day longer than 14 hours in any 7 day period
    • 10 hours’ time away from site after working 14 hours or longer
    • Provide safe transport to accommodation for those working 14 hours or longer

Don't forget to cover travel to & from 'work'. there's a lot of evidence on vehicle accidents caused by fatigue due to work.
  • Are incorparted societies a PCBU?
Whats the opinion on this scenario?
The club pay's a visiting band from the club account and charges a door entry.
  • Incident report record keeping
30 years for substance exposure - e.g Asbestos
  • Random Drug Testing
Random drug testing has been in place for many years now yet we still have high death rates in industry and little decrease in serious injuries. Is this evidence that random testing is a waste of time and money and is only promoted by those with a vested interest in making a profit from random testing
  • Tread for steel fixed ladders
    Try NZS/AS 1657:1992. sorry we don't have a copy
  • Safety statistics and classifications
    We count hours as:
Hours worked in the month/people/working days.
This gives us a number that shows how busy the place is. Overtime worked and other stress builders. This is graphed and shows when we should be vigilant around safety control measures.vc2u07uwbzr1dw9h.jpg
  • Safety statistics and classifications
    We use simple classifications.
Proactive incidents - Unsafe acts, Unsafe conditions, near miss
Reactive incidents - First Aid/Minor, Medical treatment (MTI), Restricted work (RWT), Lost Time (LTI), property damage and environmental.
Corrective actions designed to eliminate or minimise future risks.
No rewards or messages for "day's since last incident".
Recording stats is only part of the job. I believe corrective actions is where most improvement opportunity is lost. No corrective actions imposed or implemented, status quo prevails.
  • E-scooters: am I right to be worried?
I think we should all behave as WorkSafe states - Do not cause harm to yourself or others. If you wind up on ACC you get a bill for hurting yourself. if you hurt some one else you get their bill. Until you pay = denied access to E scooters. (or whatever else).
