Grant, I used to teach oxygen and LOX during my military days and is not new. Been happening for years and is usually caused due to the ingress of some form of hydrocarbon - even the smallest amount of contaminated dirt can cause an explosion. Compressed gases also generate heat or cold when in use, for example when filling other bottles from a main one the temp rises, on the other hand when you discharge CO2 it cools rapidly.
