    Just some ideas Alex. 1- Critical risk control activity (training, assessments done, team talks etc.), 2- Lessons learned from incidents, accidents etc., 3 - Improvements implemented. Also the board may be more informed and motivated by stories and actions to prevent injuries going forward rather than statistical measurement and benchmarking from the past.

    Jo Moar: Many of the so called 'HSR's' I see in companies are actually health & safety committee volunteers. Suggest reviewing if they have been formally elected by their peers to act in a legal capacity as their health & safety representative to Management. If not, you can access the unit standard and pass on anything relevant to assist their contribution and effectiveness as a committee rep for their teams.

    I accept what you say as a mantra Andrew. However, as a qualified OHS auditor, I would expect all auditors to have the integrity and knowledge to look way past the paperwork "as planned" and seek to confirm actual processes "as done" through observations and interviews. Paperwork can easily create an illusion of safety.
